Job Purpose

We are seeking an experienced Social Worker Level 3 to join an Adult Social Care service. The successful candidate will be responsible for managing a varied and complex caseload, undertaking statutory social work duties and supporting adults to achieve positive outcomes while promoting independence, choice and wellbeing.

You will work within a strengths-based approach, working collaboratively with individuals, families, carers and partner agencies to assess needs, manage risk and develop appropriate support plans.

Key Responsibilities

• Manage a caseload of adults with a range of care and support needs.

• Undertake Care Act assessments, reviews and reassessments.

• Develop person-centred support and care plans that promote independence and wellbeing.

• Complete safeguarding enquiries and take appropriate action where adults may be at risk of abuse or neglect.

• Undertake Mental Capacity Assessments and contribute to Best Interests decision-making where required.

• Work in accordance with relevant legislation, policies and professional standards.

• Identify and manage risks while ensuring the individual's views, wishes and desired outcomes remain central to the assessment process.

• Work collaboratively with health professionals, carers, families and other partner agencies.

• Support hospital discharge and effective transition between services where required.

• Make appropriate referrals and signpost individuals to community and voluntary services.

• Maintain accurate and timely case records and complete assessments and reviews within required timescales.

• Participate in multidisciplinary meetings and contribute to effective information sharing.

• Promote least-restrictive, strengths-based and preventative approaches.

• Manage complex situations and make sound professional decisions in the best interests of the individual.

Essential Requirements

• Social Work qualification and Social Work England registration.

• Relevant experience working within Adult Social Care.

• Strong knowledge of the Care Act 2014 and adult safeguarding procedures.

• Experience undertaking assessments, reviews and care planning.

• Experience of managing a complex adult social care caseload.

• Knowledge and understanding of Mental Capacity Act principles.

• Ability to work effectively with individuals, families, carers and multi-disciplinary professionals.

• Strong written and verbal communication skills.

• Ability to work independently and manage competing priorities.

• Commitment to strengths-based and person-centred social work practice.

Desirable Experience

• Experience working with complex or high-risk cases.

• Experience within community adult social care or hospital discharge services.

• Experience of safeguarding adults.

• Experience undertaking Mental Capacity Assessments and Best Interests decisions.

• Previous local authority or statutory social work experience.
