Social Worker – West Locality

Working Pattern: Hybrid
Office Days: Monday and Friday
Hours: Monday–Thursday 8:45am–5:15pm | Friday 8:45am–4:45pm
About the Role
We are looking for an experienced Adult Social Worker to join the West Locality team, supporting adults with care and support needs and their carers to achieve positive outcomes, maintain independence and improve their overall wellbeing.
This is a strengths-based, person-centred role involving assessment, care planning, safeguarding and risk management. You will work closely with adults, families, carers and partner agencies to identify needs, promote independence and ensure individuals receive the right support at the right time.
The successful candidate will have experience of safeguarding, Mental Capacity Assessments, risk assessments and reablement/enablement approaches.
Key Responsibilities

Undertake strengths-based assessments for adults and carers in accordance with the Care Act 2014.

Develop, implement and review person-centred care and support plans.

Manage a varied caseload, ensuring work is completed within statutory and local authority timescales.

Identify, assess and manage risks, ensuring appropriate safeguarding measures are implemented.

Lead safeguarding enquiries, planning and reviews where required.

Undertake Mental Capacity Assessments and apply relevant legislation and guidance.

Promote reablement and enablement, supporting adults to maximise their independence and achieve sustainable outcomes.

Advocate for adults and ensure their views, wishes and desired outcomes are central to the assessment and support planning process.

Work collaboratively with families, carers, health professionals, voluntary organisations and other partner agencies.
