Adult Social Worker – Adult Social Care
About the Role
We are currently seeking an experienced and committed Adult Social Worker to join an Adult Social Care team.
The successful candidate will work with adults with a range of care and support needs, completing assessments, coordinating support and safeguarding vulnerable adults. You will work collaboratively with individuals, families, carers and partner agencies to promote independence, wellbeing and positive outcomes.
Key Responsibilities

Complete Care Act assessments and reviews to identify individual care and support needs.

Develop person-centred care and support plans that promote independence and wellbeing.

Manage an allocated caseload of adults with varying levels of complexity.

Undertake safeguarding adults enquiries and manage risk appropriately.

Work collaboratively with adults, families, carers and multi-disciplinary professionals.

Coordinate packages of care and liaise with health, housing and voluntary-sector services.

Apply relevant legislation, policies and professional standards to casework.

Support individuals to maximise their independence and achieve their personal outcomes.

Complete Mental Capacity Act assessments and contribute to best-interest decision-making where required.

Maintain accurate, timely and comprehensive case records.

Participate in team meetings, supervision, duty work and other service requirements.

Ensure all work is completed in accordance with the Care Act 2014, safeguarding procedures and local authority policies.
Candidate Requirements
We are looking for candidates who have:

A recognised Social Work qualification.

Current Social Work England registration.

Previous experience working within Adult Social Care.

Strong knowledge of the Care Act 2014 and adult safeguarding.

Experience completing assessments, reviews and care/support planning.

Experience managing a caseload and working with complex needs.

Strong communication, assessment and report-writing skills.

Ability to work independently and as part of a multi-disciplinary team.

A person-centred and strengths-based approach to social work practice.

Enhanced DBS clearance or willingness to obtain one.
Ideal Experience
Experience in areas such as community social work, hospital discharge, safeguarding adults, mental health, learning disabilities, physical disabilities or older people's services would be advantageous.
