Mental Health Social Worker – Hospital Discharge Team

We Do Social Work is recruiting an experienced Mental Health Social Worker to join a busy Hospital Discharge Team within a North West Local Authority.

This is a fast-paced role supporting patient flow from psychiatric inpatient units, reducing delayed discharges and ensuring adults are safely transitioned into the community with the appropriate support in place.

The Role

As a Mental Health Social Worker, you will:

• Support the timely and safe discharge of adults from psychiatric inpatient settings.
• Complete strengths-based assessments and discharge planning.
• Coordinate move-on arrangements from hospital discharge and step-down placements.
• Work collaboratively with ward staff, NHS colleagues, CMHTs and partner agencies to facilitate safe discharges.
• Manage complex mental health cases within a fast-paced environment.
• Liaise with service users, carers and families to ensure person-centred outcomes.
• Maintain accurate case records and work in accordance with the Care Act, Mental Health Act and Mental Capacity Act.

Essential Requirements

To be considered for this role, you must have:

• Social Work England registration.
• A recognised Social Work qualification.
• Recent experience working within a Mental Health Hospital Discharge Team or a Community Mental Health Team (CMHT).
• Experience managing complex mental health cases and coordinating hospital discharges.
• A good working knowledge of the Care Act, Mental Health Act and Mental Capacity Act.
• Excellent assessment, risk management and multi-agency working skills.
• The ability to thrive in a busy, fast-paced environment.

Please note: Applicants without recent Mental Health Hospital Discharge or CMHT experience will not be considered.
