About the Role
We are seeking an experienced Adult Social Worker to join an integrated and supportive Adult Social Care service in Wirral.
The successful candidate will work with adults, carers and families with a range of complex and long-term needs, supporting people to achieve greater independence, choice and control. You will work within a strength-based and personalised approach, ensuring practice is underpinned by the Care Act 2014, safeguarding principles and social inclusion.
You will also have the opportunity to support the development of colleagues, including newly qualified social workers, AYSEs and Care Navigators.
Key Responsibilities

Undertake Care Act needs-led assessments, reviews, care and support planning and risk assessments.

Work with adults experiencing complex physical health, mental health and social care needs.

Support adults, carers and families to identify their strengths, assets and desired outcomes.

Promote choice, independence, autonomy and social inclusion.

Undertake safeguarding enquiries where adults are experiencing or at risk of abuse or neglect.

Complete Best Interest Assessments in relation to Deprivation of Liberty Safeguards.

Participate in the duty function, including case triage and regular team huddles.

Work collaboratively with Multi-Disciplinary Teams (MDTs) and external partner agencies.

Support MASH, MARAC, MAPPA and other safeguarding processes where required.

Coordinate referrals and signposting to appropriate services.

Organise and chair complex multi-disciplinary meetings, strategy meetings and professional case conferences.

Communicate complex and sensitive information clearly to adults, carers, families and professionals.

Prepare funding applications and work with commissioners and health colleagues where appropriate.

Ensure advocacy and appointeeship arrangements are considered in line with individuals' needs and rights.

Maintain accurate and timely case records in accordance with organisational standards and information governance requirements.

Participate in community initiatives and contribute to positive social work practice.

Support the learning, development and supervision of colleagues and less experienced practitioners.
Essential Requirements

Recognised Social Work qualification such as a Degree in Social Work, DipSW or equivalent.

Current registration with Social Work England.

Demonstrable post-qualification experience in Adult Social Care.

Experience undertaking assessments, support plans, risk assessments and reviews.

Experience working with vulnerable adults, families and carers with complex needs.

Strong knowledge of the Care Act 2014, Mental Capacity Act and Mental Health Act.

Experience of statutory social work including assessment, safeguarding and complex casework.

Strong understanding of strength-based and outcome-focused practice.

Excellent communication, organisation and case management skills.

Ability to manage fluctuating caseloads and prioritise competing demands.

Ability to work effectively within multi-agency and multidisciplinary environments.

Good IT and case recording skills.
Working Arrangements
The role offers a hybrid and flexible working approach, with the ability to work across different locations within the borough according to service requirements.
The successful candidate must be able to travel across the borough and may occasionally be required to work outside traditional hours, including evenings or weekends.
The role may involve community visits, lone working and working with individuals who may display challenging or aggressive behaviour.
