We are looking for a passionate and dedicated Start Well Family Practitioner to join a supportive Early Years team, working directly with families to improve outcomes for young children during the crucial early stages of development.
This rewarding role focuses on supporting parents and carers to create positive home learning environments, helping children reach key developmental milestones and ensuring emerging needs are identified and addressed at the earliest opportunity. You will play a vital role in reducing developmental inequalities and empowering families to support their child's learning, communication, emotional wellbeing and overall development.
Working directly with families in their homes and communities, you will build positive relationships, provide practical advice and guidance, and deliver targeted early intervention support to prevent concerns from escalating.
Key Responsibilities
- Undertake home visits with families of children aged 18 months.
- Complete developmental screening and assessments, including the WellComm Speech and Language Assessmentand ASQ-SE (Ages and Stages Questionnaire – Social Emotional).
- Support parents and carers to understand their child's developmental progress and identify areas where additional support may be beneficial.
- Provide tailored advice, guidance and practical strategies to enhance the home learning environment and promote positive child development.
- Deliver strengths-based interventions designed to improve communication, social, emotional and developmental outcomes.
- Identify emerging needs and coordinate appropriate support and referrals where required.
- Build trusting relationships with families to encourage engagement and positive change.
- Carry out follow-up visits and review progress against agreed actions and recommendations.
- Maintain accurate and timely case records, reports and outcome information.
- Record all interventions and visit outcomes using internal systems and the NHS Foundation Trust's Electronic Patient Record system.
- Work collaboratively with health professionals, early years settings and partner agencies to ensure families receive the right support at the right time.
What We're Looking For
- Experience working with children, families or within an early help, family support, health visiting or early years setting.
- Knowledge of child development and the importance of early intervention.
- Excellent communication and relationship-building skills.
- Ability to engage and support families from a range of backgrounds.
- Experience completing assessments, recording outcomes and maintaining accurate case records.
- Understanding of safeguarding and child protection procedures.
- A compassionate, child-centred and strengths-based approach.
