Adult Practitioner
Location: Telford & Wrekin
Working Arrangement: Hybrid – Office based on Mondays, with home and community-based working
We are seeking an experienced Adult Practitioner to join a busy and supportive Adult Social Care team in Telford & Wrekin.
This role offers the opportunity to make a meaningful difference by supporting adults to maintain their independence, improve their wellbeing, and access the support they need through strengths-based and person-centred practice.
Key Responsibilities
- Complete Care Act assessments, reviews and reassessments
- Undertake Carers Assessments and Mental Capacity Assessments
- Contribute to Best Interest decision-making processes
- Develop outcome-focused support plans that promote independence
- Identify and manage risk, supporting positive risk-taking approaches
- Support safeguarding activity and contribute to service improvement
- Work collaboratively with families, carers, health professionals and partner agencies
- Promote access to community resources, assistive technology and personal budgets
Requirements
- Experience working within Adult Social Care
- Strong assessment, review and care planning skills
- Knowledge of the Care Act, Mental Capacity Act and safeguarding procedures
- Enhanced DBS with Adults and Children's Barred List
- Full UK Driving Licence and access to a vehicle
